Q- Where can I find the records office?
A-The records office is located at window 3, main building of Baguio City National High School (BCNHS).

Q-At what time does the records office operate?
A-The office is open from 8 am to 12 noon, 1pm to 5pm, Monday to Friday.

Q-How can I claim a copy of my FORM 137 or Student’s Permanent Record
A-For transferee to other school, a request letter from requesting school is necessary to claim a copy of your FORM 137 or Student’s Permanent Record. The client can also request a copy of the FORM 137 for reference purposes, ie. abroad, passport, late registration, etc.

Q-How do I get a second copy of my FORM 138 or Report Card
A-In securing a second copy of FORM 138 or Report Card, a clearance should be accomplished. Students whose FORM 138 has already been forwarded to other college or university can no longer be issued a second copy of FORM 138 or Report Card, instead should secure transfer credentials from last school attended.

Q-What else can I request from the office?
A-Other requests include Certificate of Graduation, Certificate of Enrollment, Certificate of English as Medium of Instruction, Good Moral Certificate is issued at the Guidance Office for currently enrolled students and fresh graduates.

Note:
For easy retrieval of school records, please know your year of graduation or last school year attended at our school.

  3. gud am poh.. ndi po ba pwedeng magrelease ang registrar ng form 137 once nakapgrelease na po sila



  4. ellow po pano po pag nawala sa taxi ung forms at grades (card) pano po makaka kuha ng copy ng form 137?



  5. ask ko lang po bakit wala na daw dyana ng high school diploma ko, diko pa naman po nakukuha.pano po ba magrequest ng bago?nandito kais ako sa Uk ngayon. pls help…my name is josephine a. lopez,batch 1990…

  6. If you haven’t claimed your diploma, it must be in the records section. If it’s lost, go to the Records Office with an Affidavit of Loss.



  7. Please personally go to the Records Office.
